Community Health Assessment Final Survey Available

The Final Community Health Assessment (CHA) is open for the public to participate in. This survey is anonymous participation (we will not know who each survey belongs to). This FINAL CHA survey, takes a more in-depth approach at highlighting the concerns, complaints, deficits AND praises, related to health in OUR community.

This survey is intended for all individuals who consider Broadus as a central hub for their needs, you DO NOT have to be a Powder River County resident to participate.

The intentions are to compile the findings of both surveys, make improvements where possible, educate where needed, and implement realistic changes to our community, in those areas of concerns and deficit.

Once completing the Survey you will have the option to enter personal information on a separate form, to be put into a drawing for the chance to win $100 to a local establishment.

Assistance with taking the CHA survey will be available at the Clinic or Public Health Office. As well as, a paper/pen option for individuals who wish to utilize that form.
